Question: Van hesitates then jerks forward from stopped position

I have a 93 Ford e250 xl 4.9l in-line 6. And drives normal at first but after driving for a little awhile , when I come to a traffic light or stop sign it will hesitate when I start to accelerate then it will jerk forward, once it’s moving it drives and shifts normal it’s only from a stopped position after been driving for a bit.
